Support At Home Program
HCP Eligibility
Australians aged 65+ (50+ for Aboriginal or Torres Strait Islander peoples) who require assistance to live independently at home.
HCP Funding Levels
There are four Home Care Package levels, each offering a different annual government subsidy depending on assessed care needs.
Covered Costs
HCP funding covers care, home help, and technology that support safe, independent living. Items must match your care needs and plan.
If eligible, you’ll be referred for a free assessment by an Aged Care Assessment Team (ACAT) to determine your care needs and package level.
Receive Your Outcome & Choose a Provider
If approved, you’ll receive a referral code linked to your Home Care Package. You can then choose a provider and discuss how services like Vitara CarePilot can be included in your personalised care plan.
National Disability Insurance Scheme (NDIS) Eligibility
NDIS Eligibility
You must be under 65 and have a permanent and significant disability that impacts your ability to take part in everyday activities. You’ll also need to meet Australian residency requirements.

Vitara CarePilot sensors may be funded under the NDIS as low-cost assistive technology, typically supported through your Core – Consumables budget.
Covered Costs

How to Apply
Step 1: Check your eligibility
To access the NDIS, you must be under 65 and have a permanent, significant disability. You can check your eligibility using the official NDIS checklist.
Step 2: Speak with Your Plan Manager or Support Coordinator
Talk to your plan manager or coordinator about adding assistive tech like Vitara CarePilot to your plan.
Step 3: Add Vitara CarePilot to Your Plan
Vitara CarePilot sensors can be included as low-cost assistive technology, funded under your Core – Consumables budget. For low-cost items (under $1,500), no quotes or assessments are usually required.
Eligibility for Both Support At Program and NDIS Programs
Dual Eligibility
If you start on the NDIS before 65, you can keep receiving support as you age, even while applying for aged care services like HCP. However, once you move permanently into residential aged care after 65, your NDIS support ceases.
Support Coordination
Your NDIS support coordinator can help you plan and transition smoothly into HCP services when approaching age 65.
Covered Costs
Having both NDIS and HCP can expand your support, covering assistive tech, personal care, and home modifications. Together, they work to enhance independence and safety
